Employees were ordered to return to collapsed trench in fatal Texas incident, Fed-OSHA finds

Two employees of an Austin, Texas contractor who had escaped the collapse of of a 13-foot-deep trench near Creedmoor, Texas in October 2021 were told to return to the trench and finish the job, when it collapsed again, fatally injuring one of them and partially burying the other, according to Fed-OSHA. The agency cited the company for willful violations including: failing to have a trench protective system in place; exposing workers to cave-in hazards; failing to inspect the excavation; and exposing workers to the dangers of being struck by material and equipment. Fed-OSHA issues citations after employee’s death at Wisconsin factory

Following the death of a 30-year-old employee in a machine-related industrial accident last October, an Eau Claire, Wisconsin factory has been fined more than $30,000 by Fed-OSHA, which says the manufacturing line where he worked also lacked safety guards to prevent employees from reaching into the machinery while it was in operation. Wisconsin construction dies in wall collapse

A 42-year-old man was killed in an accident at a DeForest, Wisconsin construction site when a framed wall collapsed on him as he and his crew were placing it. Fed-OSHA is investigating. DeForest Times-Tribune
