Warehouse accident kills Indiana teen

An 18-year-old man died in a workplace accident on July 2 at a Greenwood, Indiana grocery warehouse. IOSHA officials are investigating. The accident occurred as the teenager was loading a truck in the warehouse and a dock plate came down on him. Daily Journal

 

North Carolina zoological park fined following fatal lion attack

Late last month, OSHNC issued the Conservators Center in Burlington, North Carolina three serious citations, and total penalties of $3,000 following the investigation of a deadly lion attack in December. The incident occurred as a 22-year-old, unpaid intern was cleaning a cage. Cited violations include failure to regularly inspect and repair the animal enclosure. Fed-OSHA orders trucking company reinstate driver

Fed-OSHA has determined an Ohio trucking company violated federal law when it fired a driver who refused to drive in hazardous winter road conditions. The agency ordered the company to pay more than $31,000 in back wages and $150,000 in damages. Cincinnati Enquirer
