Fed-OSHA fines Massachusetts plant where employee died of burns in flash fire
An Attleboro, Massachusetts company where a 26-year-old employee died in November of burns suffered in a flash fire has been fined more than $47,000 by Fed-OSHA. An investigation determined he was working with a chemical used during a cleaning process when the substance accidentally ignited, causing an explosion and triggering the building’s sprinkler system, according to fire officials. Temporary stairwell collapses at New Jersey worksite, injuring two
Two men were hurt Wednesday when a temporary wooden stairway collapsed at a condominium construction site in Essex County, New Jersey. Responding officers found a 54-year-old man had fallen about 20 feet after the stairway broke, was lying on a first-floor landing and complaining of pain in his ribs. Tampa utility TECO admits it didn’t hold safety briefing with workers before deadly explosion
Tampa Electric Company has admitted to failing to hold a pre-job briefing with workers tasked to remove debris from one of the company’s slag tanks at an electrical plant before it exploded in June 2017, resulting in the deaths of five people. Had that safety meeting occurred, the employees would have learned of procedures for water-blasting work, according to the Department of Justice. WTSP
