OSHA Investigates Death by Wig Wag

• The Occupational Safety and Health Administration is launching an investigation after a dead worker is identified.
• Kumba Tire in Macon Georgia, reportedly has a history of safety violations including those that are serious and repeat violations.
• The worker was killed when he was working on a piece of equipment called a “Wig Wag.”
• The injury occurred because the wig wag was not locked and tagged out, a fairly frequent cause of injury. The incident was reported shortly after it happened.

• A Connecticut construction contractor is looking at thousands of dollars in fines after a pier collapse.
• The Occupational Safety and Health Administration is investigating Mohawk Northeast Inc. after a pier collapsed next to a Massachusetts river, sending two workers to the hospital.
• OSHA found major safety issues including unsafe walking surfaces.
• The company is facing proposed fines for at least one serious and willful and several serious safety violations for potential fall and drowning hazards.

WA Work Zone Deaths Reach New Highs

• Fatalities in Washington State work zones are higher than ever, says the Washington Department of Transportation.
• The number has doubled from 2022 to 2023.
• The 2023 there were 10 fatalities and 28 workers seriously injured.
• At least one worker was hit and killed by a driver under the influence in 2024.

• A worker was shot at a Chicago Wendy’s by an angry customer.
• The 20-year-old individual was working the fast-food line when a customer started arguing with her, over what is not known.
• The assailant shot her and took off. She is recovering. Police are still searching.

Workers at Risk from Pesticides Safety Group Says

• The Environmental Protection Agency has issued a warning to farm workers saying that a certain pesticide is dangerous to farm workers. And safety experts are fuming.
• The EPA went to great lengths to point out that the pesticide was a danger to workers and their families thinking that the warning was sufficient to protect workers. But the chemical should not be present, the group says.
• EPA is failing by not adequately regulating the chemical saying it’s even dangerous when protective equipment is being used.
• EPA’s cumbersome process for canceling certain chemicals contributes to the problem. And allowing companies to address the problems slows the further.
