Florida rehab center facing more than $200K in OSHA penalties

  • Pensacola Teen Challenge Men’s Center is facing almost $229,000 in penalties after being cited by the federal Occupational Safety and Health Administration.
  • The agency says it found 25 safety and health violations at the non-profit’s facilities in the city, including amputation hazards, three of which have been classified as willful.
  • Teen Challenge runs a screen-printing operation, detail and wood shops, and thrift stores. The willful violations allege failure to guard hand-fed ripsaws, routers and abrasive wheels.
  • The organization’s mission is to help men 18 and older who are struggling with addition.

Feds investigating Seattle barge fatality

  • Fed-OSHA is investigating the death of a 53-year-old worker who was killed on the Duwamish River in Seattle, Washington.
  • Francisco Rodriguez Aquinaga died from head injuries after the scissor lift he was working from tipped over.
  • Aquinaga was found on the floor of the barge next to the almost fully extended 26-foot lift.
  • A coworker who witnessed the incident said that Aquinaga was welding a beam on the barge when the lift toppled, and the employee was not “strapped in” to the lift.
