Multiple offenses after man falls from Ambassador Bridge 

  • Michigan OSHA (MIOSHA) issued multiple citations after a worker fell off the Ambassador Bridge in July.
  • An employee on the bridge was completing work on the American side when he fell approximately 150 feet into the water below. The man was rescued and has since made a full recovery.
  • The day after the accident, MIOSHA began an investigation at Structural Technologies LLC, the contractor working on the span, and resulted in a total of six citations for alleged offenses.
  • MIOSHA issued a $29,400 fine to Structural Technologies LLC, which has appealed the penalty.

Fed-OSHA enters alliance with crane operators

  • OSHA signed an agreement with the National Commission for the Certification of Crane Operators and the NCCCO Foundation to protect workers who operate cranes better.
  • The two-year agreement will provide certified crane operators with training and resources to reduce and prevent exposure to the four construction workplace issues that contribute to the majority of hazards in the industry: falls, caught-in or between objects or machinery, struck-by objects or equipment, and electrocution.
  • Alliance participants will use data and review trends and incident information to develop safety alerts for employers, crane operators and others working around cranes.
  • Participants will also assist employers and operators in complying with OSHA regulations and develop relationships between the CCO’s test sites and OSHA’s field offices to provide training opportunities for crane certificants and OSHA personnel.

More severe penalties for offenders in Nevada

  • Nevada OSHA published a list of circumstances, including citations issued related to a Fatality/Catastrophe (FAT/CAT), or inspections unrelated to FAT/CAT incidents, that will cause employers to be labeled as severe violators.
  • A FAT/CAT inspection is where a citation is issued for at least one willful or repeat violation, or where NV OSHA issues a failure-to-abate notice based on a serious violation directly related to an employee death, or to an incident causing three or more employee hospitalizations.
  • Any inspection resulting in at least two willful or repeat violations; any inspection where NV OSHA issues failure-to-abate notices based on the presence of serious violations.
  • Employers are added to the  Severe Violators Enforcement Program at the issuance of a citation, meaning employers may be subject to the penalties of the SVEP program even if the employer contests the alleged citation and before the citation even becomes a final order.

Companies fined after workers injured at Lambeau Field

  • Two Wisconsin companies have been fined by OSHA in connection to the death of a worker at Lambeau Field back in mid-June.
  • Miron Construction Co., Inc. and Mavid Construction Services were both fined over $10,000 for the death of a Mavid worker who became trapped while working on the north scoreboard at Lambeau Field.
  • OSHA records show that eight serious citations were issued, four to Miron and four to Mavid.
  • Miron was fined $18,976, and Mavid was fined $15,626 for the incident.
