Wall falls on Massachusetts workers

  • OSHA is investigating after two construction workers were flown to the hospital with traumatic injuries after a wall fell on them on Cape Cod, Massachusetts.
  • First responders received a report of a wall falling on a construction worker at 513 Brick Kiln Road around 2:30 p.m.
  • Both workers were injured by a wall that had fallen on them while it was being raised and they were removed from under the wall by fellow workers before the fire rescue department.
  • The two workers were evaluated and stabilized for transfer to awaiting MedFlight and transported to the area Trauma Center.

Fines after caregiver slain at health facility 

  • McCarthy Place, which is run by Cascadia Health in Oregon, was fined $7,250 by the Oregon OSHA for failing to protect employees from workplace violence after a mental health aide was stabbed to death on July 16.
  • A recent several-hundred-page inspection report revealed that several safety violations had taken place at the duplex-style independent living facility known as McCarthy Place, which is run by Cascadia Health.
  • Mental health aide Haley Rogers, 26, was working alone on an overnight shift when she was attacked by 59-year-old James Calvin Smith.
  • Smith was a resident of the facility who had been deemed dangerous and psychotic as far back as the late 1980s and had a history of violent attacks, including an assault charge that involved the use of a weapon in 2001.

Worker dies at NJ construction site

  • A 40-year-old man died after falling four stories at a construction site in Jersey City.
  • OSHA is investigating the fatal accident that occurred on 859 Communipaw Ave. at about 10:44 a.m.
  • The employee was working for Mac Construction LLC based in Newark, which currently has another open OSHA case under investigation.
  • The agency has up to six months to complete its investigation process. No other injuries or fatalities were reported from this incident.
