Investigation opens after Orlando worker falls to death
- OSHA has initiated an investigation after a construction worker, identified as 32-year-old Javier Jimenez, fell to his death from an Orlando apartment building roof.
- A witness heard a loud “thump” and later found the roofer face-down at the corner of a breezeway with blood on his head.
- Jimenez had fallen from the roof onto the sidewalk and he was pronounced dead at a local hospital.
- OSHA declined to provide further information while the investigation is ongoing.
Canadian firm fined $50,000 after worker injured
- Lakeridge Developments Limited of Port Carling, Ont., was fined $50,000 after a worker was injured after falling 30 feet from a roof at a construction project.
- Paul Kritski, the supervisor of Peak Roofing Services Inc., was also fined $15,000.
- Lakeridge Developments was constructing a cottage, guest cottage and boathouses and subcontracted Peak Roofing to install the roofing.
- A worker from Peak Roofing Services was on a flat section of roof, when they fell 30 feet to the ground below and suffered the injury.
Missing South Carolina man died after falling into a plastic shredder
- A Spartanburg County, S.C., coroner found Duncan Alexander Burrell Gordon apparently fell into the plastic shredder where he was last seen on May 5.
- Dried blood found at a recycling plant is that of a 20-year-old reported missing two months ago at a recycling plant where his father is a supervisor.
- DNA recovered just outside the machine matched that from Gordon’s parents.
- The South Carolina OSHA and the Spartanburg County Sheriff’s Office are both investigating.
Companies cited for a bridge collapse in Georgia
- A federal investigation found that two companies failed to follow safety standards in an October 2021 bridge collapse that killed a worker.
- B&D Concrete Cutting Inc. of Atlanta and Georgia Bridge and Concrete LLC of Tucker did not conduct a proper engineering survey as an access road bridge was being dismantled.
- During the collapse, a concrete saw weighing more than 1,700 pounds struck and fatally injured the worker employed by B&D Concrete Cutting and a second B&D worker suffered injuries and was hospitalized.
- OSHA proposed penalties of $31,283 for Georgia Bridge and Concrete and $25,669 for B&D Concrete Cutting.
