No cause found for ignition of fatal Colorado gas site blast

Fed-OSHA fines Georgia warehouse company $192K
A Buford, Georgia distribution center has been fined more than $190,000 for cited violations including blocked warehouse emergency exits. The facility also lacked emergency exit signs in required positions, a repeat offense, according to the agency.
