Fed-OSHA investigates northern Colorado trench collapse that killed two

Two men who had been installing sewer pipe Tuesday for new housing in Windsor, Colorado died after being trapped in a collapsed trench last week. Both workers had succumbed to their injuries by the time emergency crews reached them, according to first responders. Fed-OSHA is investigating. Greeley Tribune

 

IOSHA fines coatings facility $14K for fatal machinery incident

Indiana OSHA has fined a Jeffersonville manufacturer of protective coatings for steel, where a 49-year-old worker died in February, $14,000 for two cited serious violations in connection with the incident. The agency determined the machinery the employee was operating was not properly guarded to prevent employees from getting caught during its cycle. Additionally, the worker was wearing a long-sleeved shirt and had unrestrained hair at the time of the incident. News and Tribune

 

Nebraska worker fatally crushed by tractor

A 64-year-old Omaha-area man was crushed to death Thursday by the wheel of a tractor after the vehicle lurched forward while he was working on it. Fed-OSHA is investigating. Omaha World-Herald

 

Victim of fatal workplace accident at Nebraska chicken plant identified

Authorities on Friday identified the 56-year-old construction worker who was killed in a 40-foot fall at a chicken processing plant last week in Fremont, Nebraska. Another worker was trapped 30 feet above the ground before he was rescued. Fed-OSHA is investigating. Hastings Tribune

Man killed in fall at Madison construction site identified

A 24-year-old man who fell at a Madison, Wisconsin construction site Wednesday and died shortly after he was transported to a hospital was identified by authorities. Officials said there were no witnesses to describe where he fell from or whether he hit any structures, equipment or objects before reaching the ground. Fed-OSHA is investigating. Worker killed in North Carolina road construction accident

A 37-year-old self-employed truck driver, who was attempting to remove straps from a load of soundproof wall slats on a road construction project in Winston-Salem, North Carolina, died Monday after a piece of the wall fell on him. OSHNC is investigating. Winston-Salem Journal
