Fed-OSHA cites tire plant after investigating injuries of three employees
Fed-OSHA has issued a tire manufacturer two repeat violations, 16 serious and five other violations after an investigation following two separate injury incidents on consecutive dates at the company’s southern Illinois plant. Three employees suffered severe injuries in incidents associated with a mixer containing a combustible dust and a flammable gas, guarding moving equipment and the failure to implement and enforce procedures to control the unintentional energization of equipment during servicing in November 2021. Some Hawaii firemen say they are living in huts
Fed-OSHA is investigating complaints made by firemen at the Army’s Pohakuloa Training Area on Hawaii island who report they were using breathing apparatus and N95 masks without proper fit testing, the HVAC system wasn’t removing gases from the trucks, and safety rules were not being followed. They also say they’ve been living in old Quonset huts since 2016, unable to move into a renovated fire station. Hawaii News Now
N.C. Labor Department presents annual safety awards to employers
The North Carolina Department of Labor recently recognized employers and employees at the agency’s annual safety awards banquet in Statesville. Companies must have been free of fatal accidents at the site for which the award is given to be eligible. Iredell Free News
Milling company and officials charged in 2017 grain explosion
A federal grand jury in Madison handed down an indictment last week alleging Didion Milling Inc. and company leaders failed to regularly clean dust accumulations from inside the mill that ultimately lead to a combustible dust explosion that killed five employees and injured more than a dozen others who were working in the mill in late May 2017, in violation of Fed-OSHA and Environmental Protection Agency regulations. Fed-OSHA cites reality TV show after former contestant’s stunt accident
Fed-OSHA has issued two citations to America’s Got Talent after a former contestant ended up in a wheelchair after a 30-foot fall during a stunt earlier this year. The incident happened when the contestant tried to escape a straitjacket while he hung upside down between two cars. The crew members released the cars early, causing him to be crushed between the vehicles as they caught on fire. TVShowsAce
