Oklahoma City window washing incident likely human error: Expert
A window cleaning safety expert says windspeeds atop a 50-story building in downtown Oklahoma City on May 15, when two window washers were rescued from a wildly swinging work basket, were too high for safe usage of the machine and that the incident was the reuslt of human error. TOSHA cites city after employee fatality
Tennessee OSHA has handed down six violations to the city of Spencer following the death of a utility worker who was part of crew that responded following a water line on a highway culvert project in April. While digging, the trench collapsed, fatally trapping the worker. The violations issued to the city included not using a ladder or safe means of egress in the trench, not providing adequate precautions to protect employees, and inspections not being made by a competent person. OR-OSHA cites school district for failure to report injuries to staff
The Salem-Keizer School District has been cited by Oregon OSHA for failing to report or investigate staff injuries caused earlier this year by students at an elementary school. The investigation found the school and district did not investigate several injuries that led to time loss, including an incident in January in which a student ran over a teacher with a cart and threw books at her.
