Acrobat injured in fall at Cape Cod fair

An acrobat was injured at the Barnstable County Fair last week in East Falmouth, Massachusetts, when he fell off the “Wheel of Death,” a large rotating structure with rotating wide hoops on both ends. The acrobat was performing in one of the hoops when he fell and hit the top of the other hoop before falling about 20 feet to the ground, according to local officials. Fed-OSHA was notified. Employee crushed by falling pipes at New Jersey warehouse

A man died at a warehouse in Clifton, New Jersey, after an industrial accident Thursday morning. A truck driver who was making a delivery says he witnessed the man unloading a bundle of pipes from a tractor-trailer when the load fell on top of him. Fed-OSHA is investigating. North Jersey

 

Kentucky OSHA investigating ice cream plant explosion

Local fire officials expect to hand off the investigation into the explosion last week at the Dippin’ Dots facility in Paducah to Kentucky OSHA. As of late last week, nine of the 10 injured people had been released from a local hospital. The explosion was the second in two years at the plant, which uses liquid nitrogen to quick-freeze its product. Paducah Sun

 

Teenager drowns at Ohio water park

A teenager died while hospitalized last week after disappearing under the water for 30 minutes at a Madison Township, Ohio water park before lifeguards retrieved her. The incident happened n the area of the park’s inflatable obstacle course. The local sheriff says he plans to contact Fed-OSHA and the attorney general’s office for assistance during the investigation. WCPO

 

MNOSHA launches investigation into death of employee at home improvement store

A Golden Valley, Minnesota Menards employee who died Thursday after an accident at the store has been identified as a 19-year-old man. First responders learned he died when a pallet of lumber fell on the forklift he was operating. MNOSHA said there hadn’t been any safety inspections at the store in the past five years. KSTP

 

Purdue, Fed-OSHA workshop to help prevent grain dust combustion, explosion

Funded by a grant from Fed-OSHA, a Purdue Extension workshop in September at the Southern Indiana Purdue Agricultural Center will help farmers and those who work in grain handling facilities better understand how to prevent grain dust combustion and explosions. Dubois County Free Press
