Fed-OSHA issues third-highest fine against Ohio employer since 2015
Fed-OSHA has fined Fuyao Glass America more than $724,000 for nine alleged repeat and 13 alleged serious violations, including exposing employees to electrical safety violations and failing to evaluate the workplace to determine permit-required confined spaces and train employees on lockout/tag out and entering confined spaces, including other hazards. Report links highway construction hazards to deadly 2017 Texas incident
A Fed-OSHA report finds that serious hazards near Interstate 10 near El Paso led to a crash in December 2017 that killed two people riding in an 18-wheeler struck a concrete placer. The report indicates access to the construction site was not restricted by the contractor, and the keys were not removed from the ignition of the concrete placer when workers left the worksite. Coworker dies trying to save man who fell into truck for field seeding
Fed-OSHA is investigating after two men who had been working near a hydro-seeding spreader truck died Monday morning at a Texas high school. The first passed out from fumes and fell inside the truck. His coworker who tried to help him also was overcome by fumes. Miami Herald
Workplace fatigue is a growing problem
Fatigue is estimated to cost employers more than $136 billion annually in health-related lost productivity, and can have fatal consequences. Studies indicate 13 percent of workplace injuries are attributed to fatigue and 43 percent of workers say they sometimes are too tired to function safely at their job. Herald-News
