Kentucky: GE fined $98K related to fatal accident

Kentucky OSH has handed down $98,000 in fines GE Appliance Park in Louisville after a worker who was fatally pinned between a conveyor and a stair rail in February. Cited violations include two repeat-serious violations for failing to address issues found in foam insulation loops in an area where foam insulation is injected into refrigerator doors and baked onto the surface. Idaho tree services firm fined after fatal accident

Fed-OSHA has fined an Idaho tree services company in connection with a fatal accident in November that killed a man and severely injured another when a bucket truck fell 65 feet to the ground during a tree-trimming incident. The report said both employees were trimming tree limbs and a limb swung below the bucket, causing a side load on the knuckle boom. The boom arm broke and both employees fell to the ground. The original fine of more than $14,000 was reduced to $12,000. MagicValley.com

Fed-OSHA has fined an Eau Claire, Wisconsin manufacturing facility more than $782,000 for failure to implement and training employees on lockout/tagout procedures, installing machine guarding or complying with forklift regulations.
