
OSHA Fines Clean Harbors $600,000 in Ohio Tanker Cleaning Fatality
- OSHA has found that Massachusetts-based Clean Harbors Environmental Services failed to properly ventilate a confined space, leading to the death of a worker in a railroad tanker car on the premises of Emerald Transformer in Twinsburg, Ohio, in January.
- The agency said a railcar contained “organic chemical residue”.
- OSHA has cited Clean Harbors for violations that include three labeled as “willful” and has proposed more than $600,000 in penalties.

NJ Roofer in $155,000 Settlement of Multiple Violations
- RRC Home Improvement Inc. has settled litigation with the U.S. Labor Department stemming from investigations last summer that found the Newark, N.J. roofing contractor repeatedly exposed workers to fall and safety hazards at two worksites.
- The first investigation in June 2024 followed reports of employees working on a roof without fall protection.
- A month later, investigations began at another site as part of the agency’s National Emphasis Program for Falls in Construction.
- There, OSHA inspectors again observed employees working without required fall protection and uncovered violations involving lack of hard hats, eye protection, and fire extinguishers, as well as non-compliant pump jack scaffold poles and unsafe ladder use.
- The settlement agreement between OSHA and RRC affirms the citations issued after last year’s inspections, which included four willful and seven serious violations. The company also agreed to pay a $155,000 penalty.
